The mission of the Central Savannah River Area Partnership for Community Health, Inc. (The Partnership) is to improve the health status of persons in our community through the development of a shared vision for community health. By health status we mean the spiritual, economic, environmental, social, psychological and physical health status of our community.

The Partnership is a group of seven hospitals, two health departments and 42 representatives from the social service, educational, business, governmental and religious communities.

- Explore needs of our community
- Establish projects to meet those needs
- Develop funding for the continuation of the projects

In collaboration with the University Hospital Healthcare System and the Beulah Grove Community Resource Center we were able to achieve the following:
- Provide an average of 500 patient visits per month to low-income residents in the 30901 zip code area at the Center for Community Health, a primary care health center
- Provide low-income (uninsured/underinsured) patients with reduced price medications
- Develop and implement community plans with residents in the 30901 zip code area
- Provide after-school services to more than 300 youth
- Provide more than 500 youth with violence prevention classes and activities
- Provide more than 100 youth with field trip opportunities
In collaboration with the University Hospital Healthcare System, the Neighborhood Improvement Project , Care Management Consultants and residents in the 30906 zip code area we were able to achieve the following:
- Provide an average of 600 patient visits per month to low-income residents in the 30906 zip code area at the Neighborhood Health Services Center, a primary care health center
- Develop and implement community plans with residents in the 30906 zip code area
- Develop youth tutoring programs to more than 100 youth
- Developed eldercare case management programs which provide services to elderly residents
- Developed an eldercare transportation program which provides seniors with transportation services
